The Case Manager is responsible for the ongoing development and provision of housing services for Transitional Housing and Rapid Rehousing program participants to assure general program compliance and participant success. Responsibilities include: conducting housing assessments for eligible program participants and prioritizing housing offers based on risk; assisting eligible program participants in locating housing in Lawrence County and beyond; building and maintaining strong professional relationships with local landlords; delivering housing counseling and safety planning services for adults, children and families; representing the agency by participating in local, regional and state housing coalitions; and, maintaining client files and data entry systems including, but not limited to, case notes, service plans, and other important program participant documentation as required by funders.
